WAIVER & RELEASE: Hudson Valley Rivermen Basketball, Inc., The County of Sulllivan, The Town of Fallsburg, & Fallsburg Central School District facility will not assume liability for injury or damages sustained as a result of the above named Athlete's participation unless due to willful misconduct or gross negligence on the part of the Hudson Valley Rivermen, its affiliates or agent. Due to the strenuous nature of basketball, all athletes are urged to consult with their physician concerning their health and phyiscal well-being prior to participation. Athletes will be required to fill out a medical waiver on the day of Tryouts. Basketball presents certain inherent risks and hazards, which the participting Athlete is urged to consider.

I hereby consent to all the terms written in this document and to the best of my knowledge, and agree that there are no physical conditions which will interfe wit my participation to play basketball.
